I recently found out the reason for this: it's the Quality Outcomes Framework. It is literally the case that doctors are paid to nag people over a certain BMI in certain prescribed ways. And you may notice that you get a lot of calls and letters from your doctor to come in in March because the numbers are calculated at April 1 and they need the cash.
(The QOF is where that strange government meme about "greedy doctors" comes from: the QOF was put into place anticipating 75% payout, and the first year it was a 90% payout and the second it was a 95% payout. i.e., they're whining that the incentive scheme worked too well. So now the QOF outcomes are being tweaked to make the game harder, thus doubly distancing them from anything that's actually good for patients. Yay politics!)
